Therapists
Dental Therapists are required to be registered under the
Dental Practice Act 2001. The following documentation is required to be
affixed to the application:
- Fully completed application form (available by
telephoning the office)
- One passport size photograph signed by the applicant and certified
on the back by a referee or Justice of the Peace that the photograph is
a true likeness of the applicant
- A certified copy of the qualification
- Three references attesting to your personal character from
professional people (on letterhead) and not from family members, and
dated within the last three months, who has known the applicant for at
least twelve months
- Documentary proof of Professional Indemnity Insurance or a statutory
declaration supporting an exemption from that requirement. More

- Proof of identity (either driver’s licence, birth certificate, or
passport)
- The current application fee is $100 and $40 for provisional
registration
- The application must go to a Board meeting for consideration and
approval
Dental Auxiliaries (either Dental
Therapists or Dental Hygienist may also apply for registration through the
Mutual Recognition Act and the Trans Tasman Mutual Recognition
Act.
